Faith Formation

Faith Formation at St. Michael the Archangel Parish prayerfully assists parents as they fulfill the promises they made at their child’s Baptism: raise their child in the Catholic faith and guide their child to follow the Ten Commandments.

Faith Formation is offered for grades Pre-K through grade 6.

Classes meet on Saturday from 8:45 AM -10 AM.

For the Registration Form, download, print and complete pages 1-2.
The Parental Consent Form (pages 3-4) is provided in both English and Polish. Choose ONE version, download, print and sign.

Bring the completed Registration Form with payment and Parental Consent Form to Open House on Sunday, September 6 in the Polish School kitchen or have your child bring it to the first class.

Printed copies of all these forms will be available at Open House on September 6.
